intake form

(A necessary ingredient)

Bow Tie Massage takes pride in making each massage session specific to each of our client's needs. 

In order to do that, you must complete our intake form.  Among other things, the form tells us if anything on your body hurts, what type of pressure you like, if you're on any medications, etc.  This is necessary in order to make sure that you are safe and your massage goes as smoothly as possible.

Additionally, we want to make sure that you've gone over our "Policies" before starting your session.  We also need your meal, dessert and drink order. 

So, please take a moment to complete our intake form by scrolling down to the bottom of this page and filling in each box.  Then click "submit" and it automatically goes to our e-mail.

We keep our prices low and regularly offer specials, so that you can afford to come to Bow Tie several times per year. A 15-20% tip is appropriate for any of our massage packages. Please keep this in mind when choosing your package. However, the minimum that we require is 10% in order to stay open and continue to offer our services to the public.

If I arrived late, I understand that my session may have to be cut short or my package changed in order to prevent the delay of other client’s sessions.
